Using IEP Data Collection Sheets to Simplify Writing Your IEP

These IEP data collection sheets are tools you can use to prepare for upcoming IEP meetings. They can help ease your mind by letting you see you have all of the puzzle pieces there. These IEP data collection sheets allow you to put all those pieces together with ease.

Here is how I  use these IEP data collection sheets to help I put together the overload of information to form an IEP.

INPUT SURVEYS

I send home this parent input survey. This allows me to gather input from the parents and note important strengths, concerns, and additional input they have. (This is also important when it comes time for the meeting in case, they are unable to attend.)

I use this student input survey and work one on one with the student to have them answer the questions. This allows me to see what they are thinking and knowing about their own education. Even young students in lower elementary grades love doing this! I enjoy this part so much! Hearing what they want to be when they are older is one of the best parts. I was also very surprised by how honest and open the students were when it came to discussing their weaknesses and what they wanted to work on in the upcoming school year!

 One of the greatest IEP data collection assets I have used are the teacher input survey forms. They are filled out by each of the teachers that work with the student throughout the day. This help give a clear picture of how the student performs in different settings.

SEPARATING STUDENT’S MATERIALS 

Once I have my surveys, I will put those with my data collection from the student’s goals. (If you want to read more about data collection on student’s goals, check out this blog post.I will then put each student’s information in a manilla folder. As I get more information about each student it will go into that folder to help me begin writing their IEP. I will sit down with my caseload roster and see what dates the IEP’s need to be held. I will see what related services providers need to be invited from this as well.

Now that it is time for notices and envelopes I will use the handy special education labels to send them home. 

MEETING TIME

With the prep work completed, I get the IEP draft ready to go for the meeting. Now comes the stress of holding the meeting. To help ease some of the stress I use these Folder Organization forms to help the meetings run smoothly. I use a sign on the door to ensure there are as few interruptions during the meeting as possible. Next, use an agenda to keep the meeting on track. I have an outline for the minutes that need to be taken. This also helps whoever is taking the minutes assists me staying on track and making sure I do not miss anything.

AFTER THE MEETING

Once the meeting is over, the pages are signed, and the parents have their copies, I still need to put all the papers from the meeting in order. In the Folder Organization forms I mentioned earlier, there is an IEP order form. It helps me keep all my IEP paperwork in the same order. This way, if I go to look for a certain document within the IEP folder, I will know exactly where it is!

And there you have it! The IEP prep, collecting data, putting it together, holding the meeting, and putting it all back in order! Once all that is done, you definitely deserve to sit back, relax, and take a breather. Well, that is, until it is time for the next one!!
