SPED Forms help teachers stay organized. As special education teachers, it’s a word we can love and despise at the same time. Hearing the word form can make you think…great…one more thing to add to the ever-long to-do list.
On the other hand, forms can help take the mounds and mounds of data that we as special education teachers must live, breathe, and use daily.
Let’s make things easier
As special education teachers, we are supposed to be able to deliver and collect specific data about individuals which can become very overwhelming in a hurry!
The way I look at sped forms, is how can I use them to my benefit. I use it to help me organize and arrange all the madness of services, times, different service providers, and data collection. Being able to recall information about my students quickly is so important.
The Student Snapshot is one of my favorite special education forms I use. I use it as I read through a student’s IEP. I use it to help me break down the different parts of the IEP into smaller, easy to read portions. It makes me read the IEP in more depth and I make sure I am not missing anything while I am reading.
Once the Student Snapshot is complete, I put a copy in each individual student’s notebook. I also make a copy of it for the general education teachers that work with the student. Finally, I have a copy available for me in a notebook on my desk for a quick reference.

I am a hands-on learner, so writing things out on the Student Snapshot sheet really helps me learn more about my students as the year kicks off.
Another sped form I use is the Caseload Documentation form. This form lets me see my entire caseload in a snapshot. Once I fill it out, I can quickly see who receives other services.
These services include related services such as:
- speech
- occupational therapy
- physical therapy
- adaptive PE
- Orientation & Mobility
- Nurse services
just to name a few. I can quickly view the student’s contact information as well.
